Position Summary Reporting to the Manager of Dialysis, Diabetes Education, OPNC and Paediatrics, the successful applicant will be responsible for the provision of Diabetes education to the patient in the Diabetes Education program. This will include assessment of patient's educational needs, implementation of the education in the most appropriate format and evaluation of the effectiveness of the education. The successful applicant will work as a member of the Diabetes education team. Mandatory Qualifications At least 2 years recent experience working in Diabetes Education Clinic setting Graduate of a University Nursing Program (BScN) Current Basic Cardiac Life Support (BCLS) certification Registration (in good standing) with the College of Nurses of Ontario Education Requirements Certification in Diabetes Education preferred or actively pursuing Certified Diabetes Educator (CDE) Designation Certification in Insulin Pumps or actively pursuing Mandatory Competencies Demonstrated ability to provide diabetes education, including management techniques to a variety of adult patients with Type 1 and Type 2 diabetes and those with gestational diabetes using a variety of learning styles incorporating cultural knowledge and low-literacy learning Current in evidence based best practices and clinical practice guidelines Demonstraed ability to provide diabetes education, including management techniques to children with Type 1 and Type 2 diabetes Ability to engage in health promotion activities on and off site Highly developed communication, presentation and group facilitation skills Experienced in the use of insulin pumps and the continuous glucose monitoring system Exceptional interpersonal and communication skills with a team focus Self directed individual with the ability to work both independently and in collaboration with other staff members as a part of program planning team Demonstrated commitment to continuing education and personal development in nursing Creative problem solving abilities with a focus on solutions and patient friendly approaches Demonstrates commitment to exceptional customer service with patients, families, staff, colleagues and community partners with an awareness of cultural, racial, economic and social diversity Proven critical thinking skills Proven organizational and time management skills with ability to multi-task and set priorities in a rapidly changing environment Well developed negotiation and conflict resolution skills Ability to establish positive and proactive work relationships with external partners Demonstrated ability to be a coach, teacher and mentor to colleagues and students Demonstrated excellence in patient assessment and care planning Reliable individual with excellent attendance record and ability to work flexible hours during the day and early evening.
