We have a fabulous opportunity available for you to join our awesome team as our Support Coordinator. We support our brilliant customers who have complex mental health support needs. We help them live independently in the community in a flexible and bespoke way to support them to meet their goals and aspirations.

The opportunities are in our community and floating support service which we call STEP (Supporting Tenancies, Enabling People) with our customers who live in a community-based setting in the Sunderland and Washington areas.

The STEP service is successfully being developed across the Northeast and is still in its infancy in our area. This is an exciting opportunity to get involved in developing the service and supporting it to evolve.

We help change our customers lives for the better. Youll play your part by assisting in the delivery of tasks that are based on our customers personal support plans, their needs, and aspirations. Youll work closely with external agencies and professionals to assess and engage support for our customers.

We empower our customers to develop the independent living skills they need, to successfully move on. Together well make a difference!

Typical day as a Support Coordinator

• Creating support plans with your customers and working together as a team to ensure the smooth delivery of support.
• Supporting our customers with complex mental health needs, learning difficulties and/or dual diagnosis. Attending MDT's, risk planning meetings etc to enable us to best support our customers.
• Helping customers with daily living activities such as making meals, shopping, budgeting, maintaining their tenancy and moving onto independent accommodation if they are able.
• Carrying out risk assessments, support planning, goal setting and regular reviews.

Cuppas, chats and catch ups. Theres no rushing from one customer to the next here! Plenty of time to collaborate with colleagues too.

About you

• Youre passionate and motivated to help vulnerable people make positive and lasting changes to their lives. Youll impress us with your ability to work on your own initiative, positively influencing people from all different backgrounds.
• You haveexperience in safeguarding, risk assessments, confidentiality, support plans, alongside working collaboratively.
• If youre not the biggest fan of technology, thats not a problem to us. You do need to know that we are quite a high-tech company and most of our application process and mandatory learning are online. However, were glad to support you if you need help.
• To make sure our customers are well supported, its important that youve had a jobin asimilarenvironmentbefore. Dont worry, we wont drop you in the deep end, well help to you understand how things work at Home Group!
• As this is a community-based job, you need to be able to drive, as well as having access to a vehicle insured for business purposes(we pay your expenses to cover this use!)

Job details

• You will work on a rota basis, Monday-Sunday.
• Where were flexible with you, we do need to meet the needs of our customers. That may mean adapting your start or finishing times on occasion.
• Able to use technology for creating and updating support plans, complete online learning and to collaborate with colleagues.
• Youll need an Enhanced DBS check done and we pay for that.
