The Nurse Practitioner will be hired by Mount Sinai Hospital to work at a part-time Mount Sinai clinic at VOCAL-NY in Brooklyn, New York (300 Douglass Street). VOCAL-NY provides onsite and peer-based syringe exchange, overdose prevention, hepatitis C and HIV education, drug treatment education and other services for people who use drugs. Services are provided in a low-threshold, nonjudgmental, and culturally relevant manner.

Position: The Nurse Practitioner will work on a project funded by the New York State Department of Health. The VOCAL+SINAI Cures Project employs core principles of harm reduction and combines the expertise of VOCAL-NY and Mount Sinai Hospital. VOCAL+SINAI Cures implements a peer-driven, community outreach model in Brooklyn to screen, link to care, and cure persons who inject drugs of Hepatitis C Virus (HCV) infection, as well as treat opioid use disorder. Clinical care will be provided at VOCAL-NY through low-threshold onsite and telemedicine care, health promotion, and care coordination. The Nurse Practitioner will collaborate closely with general internists, addiction medicine specialists, and hepatologists at Mount Sinai Hospital who are experts in providing HCV and buprenorphine treatment.

General Summary

A nurse practitioner (NP) as described in Title VIII, Article 139 section 6910 of the New York Education Law is an RN who has earned a separate certification as an NP through advanced clinical nursing education in a distinct specialty area of practice. Nurse practitioners may diagnose, treat, and prescribe for a patient’s condition that falls within their specialty area of practice.

This position is for a graduate of New York State registered Nurse Practitioner Program or equivalent and is certified as an Adult Primary Care or Family Nurse Practitioner by a national association via examination.

Responsible To

Medical Director of VOCAL + Sinai Cures clinic at VOCAL-NY.

Responsible For

Patient care as outlined by a collaborative practice agreement or collaborative relationship.

Principal Duties and Responsibilities

Management

Oversees clinical operations within the VOCAL+ Mount Sinai Cures Program, using data and fact-based problem-solving techniques to improve processes and outcomes. Together with the Medical Director, ensures that all clinical and programmatic initiatives run according to institutional and departmental policies and in accordance with any government or regulatory requirements as applicable.

Trains, coaches, counsels, mentors, disciplines, and evaluates project staff in accordance with all internal policies and procedures. In collaboration with Medical Director, communicates values, strategies, and objectives of the program on a regular basis. Assigns accountabilities, delegates tasks and responsibilities, and plans, monitors, and appraises job performance.

Establishes and implements short- and long-term goals, objectives, policies, and operating procedures for high level of performance in programmatic care delivery.

Oversees the collection, compilation, and analysis of clinical program activity data to evaluate clinical performance and competence.

Provides technical assistance, resource services, consultation and education to patients and providers on community outreach, within the clinic and across a tele-medicine platform.

Clinical Practice

· Assesses the health status of patients/families by means of comprehensive health history and physical examination.

· Initiates selected therapeutic and diagnostic procedures, consultations, and laboratory examination, prescribes medication regimens, interprets findings, and alters plan of care according to accepted standards of care and practice guidelines.

· Educates and counsels in the areas of health promotion, maintenance, disease prevention, and management of acute/chronic illnesses.

· Documents findings and plan of care in the patient's medical record.

· Consults with physician according to established standards, policies, procedures, and protocols.

· Initiates referrals to specialty services and fosters continuity of care.

· Participates in team meetings and conferences to enhance an interdisciplinary approach to care delivery.

· Performs a patient record review with the collaborating physician in a timely fashion but not less than every three months if utilizing a collaborative practice agreement.

· Practices as a member of the allied health staff according to the rules and regulations of the Medical Staff and the bylaws of the Hospital Staff.

· Prescribes medications to treat HCV infection.

· Prescribes buprenorphine to treat opioid use disorder.

· Prescribes medications to treat alcohol use disorder.

· Provides wound care.

Minimum requirements:

1. Licensure: New York State License with current registration as a registered nurse.

2. State Certification: Certified by the New York State Department of Education as a Nurse Practitioner as follows:

a. Is a graduate of New York State registered Nurse Practitioner Program or equivalent OR is certified as a Nurse Practitioner by a national association via examination.

b. Has completed a pharmacy component of not less than the required semester hours or the equivalent AND has been instructed in New York State and Federal Laws relating to prescriptions and recordkeeping (3-hour course).

c. Has a Collaborative Practice Agreement with a physician and designated protocols if less than 3600 hours experience as a practicing Nurse Practitioner.

OR

Has a Collaborative Relationship with a physician based on employment in clinical practice in the NP role at Mount Sinai Hospital. If there is a documented work history as a Nurse Practitioner with greater than 3600 practice hours. Please note all NPs regardless of their status will be placed on an FPPE initially)

3. Education:

Master's degree program designed to prepare a NP to diagnose, treat, and prescribe for a patient’s condition that fall within their specialty area of practice or the successful completion of an approved supplemental education program, i.e., post-Masters NP certificate.

4. Experience: Previous progressive experience relevant to harm reduction, treatment of hepatitis C virus infection, and treatment of opioid use disorder with buprenorphine preferred.

5. National Board Certification in Advanced Practice Nursing required. This requirement may be deferred up to 1 year if approved by Chief Nursing Officer or designee.

6. Credentialed through the Credentialing Committee of the Medical Board with approved Delineation of Privileges

7. Drug Enforcement Administration (DEA) certificate and Basic Life Support (BLS) certification required

8. Bilingual (English/Spanish) preferred

Compensation: The Mount Sinai Health System (MSHS) provides a salary range to comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for this 22.5 hour per week role is $77.4629 - $77.4629 Hourly. Actual salaries depend on a variety of factors, including experience, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
