Job Description





An exciting opportunity has arisen for an experienced HCSW to join our Intermediate Care Team as part of the Integrated Community & Support Services (ICASS).

The Intermediate Care Service provide rehabilitation to the adult population within the locality. This will be delivered to service users in their own home or as out-patients.

The primary function of this role will be to work with the Physiotherapy Service within Intermediate Care to deliver therapeutic treatment programmes to service users in their own homes, preventing hospital admission and supporting early discharge from hospital. Therapists will provide specialist assessments to this patient group and allocate work to you. You will also be expected to hold your own caseload, assess, progress and discharge autonomously within parameters.

You will have an HNC in Health and Social Care or equivalent or SCQF level 8 e.g. Postural Stability Instructor or equivalent. Experience working in the NHS as a therapy support worker would be essential. It would be essential to have experience working with older people as they are the main group of service users referred.

Working as part of a supportive multidisciplinary team you will be expected to carry out ongoing treatment programmes for a mixed caseload of mainly older people.

You will have excellent organisational skills, excellent communication skills and be able to work independently as well as within the team. There may be occasions where you will be required to work in one of the other community hospital sites.

Public Holiday working is in operation.

Our service offers regular supervision, PDP and supports training opportunities. The ability to travel throughout the area is essential for this community post.

NHS Scotland is reducing their full-time working week from 37.5 to 37 hours per week from 1 April 2024 but with no change in pay. This reduction will also be applied pro rata for part time staff. However, as not all service areas will be able to adopt the 37 hour working week immediately from 1 April 2024, you may be required to work up to an additional 30 minutes per week for a temporary period for which you would be paid until the service you are working in changes rosters or working patterns to accommodate the new reduced working week.
