Job Opportunity: IP Pharmacist in Basingstoke
£35 - £37 per hour | Monday to Friday | GP Surgery
Are you an experienced IP Pharmacist looking for your next challenge? We have an exciting opportunity in Basingstoke to take the lead on the medicines management for a cohort of patients, making a real difference in both their care and health outcomes.
Key Responsibilities:
Lead medicines management for patients in specialized areas, including cardiovascular, frailty, pain, and mental health.
Advise on polypharmacy, suggesting alternatives and reducing wastage within the practice.
Manage transfer of care from hospitals and care homes, including medicines reconciliation, obtaining specialist supplies, and liaising with local pharmacies.
Provide expertise in practice therapeutics, supporting complex prescribing decisions.
Lead medicines reviews for both specialist and generalist cases, including for complex patients.
Develop integrated pharmaceutical care plans, tailored to individual patient needs.
Provide education and training for clinicians and prescription teams on complex prescribing issues.
Rationalise repeat prescription lists to reduce waste and promote the repeat dispensing service.
Promote self-care, health promotion, and wellbeing advice.
Develop and implement prescribing guidelines in collaboration with secondary care and CCGs.
Lead the analysis and interpretation of complex health and drug-related information.
Interpret and implement national and local medicines optimisation programmes.
About You:
GPhC registered with experience in a clinical or specialist pharmacy setting.
Strong knowledge of therapeutic areas (e.g., cardiovascular, frailty, mental health) and medicines optimisation.
Proven ability to manage complex prescribing issues and deliver patient-centered care.
Excellent communication skills to educate and collaborate with multidisciplinary teams.
Passionate about improving patient outcomes and reducing medication-related harm.
