Your Role

As a Home Health Aide, you will be responsible for providing personal care services to patients, including bathing, grooming, skin care, toileting, and oral hygiene. You will also communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.

Responsibilities

• Assist patients with lifting and transferring
• Maintain good relationships with patients, families, and staff
• Report changes in patient medical and living conditions to supervisors or case managers
• Ensure a safe and healthy living environment
• Complete documentation according to agency protocol and Medicare/Federal guidelines

Requirements

• High school diploma or equivalent
• State Tested Nurse Aide (STNA) or 75-hour nurse aide training (CNA)
• One year of experience as a nurse aide in a hospital, nursing home, home care agency, or private setting under RN supervision
• Completion of skills competency test with 80% or higher score
• Valid driver's license and auto insurance in your name as a driver
