The hiring salary range for these positions are $4,446.52 - $5,335.56/month or $2,223.26 - $2,667.78/semi-monthly depending on qualifications.

This class is the second level in a four-level Aging Services Series devoted to providing telephone and on-site public benefits application assistance to clients who specifically need to apply for Supplemental Security Income (SSI), Social Security Disability Insurance (SSDI), Aid to the Needy and Disabled (AND), Long-Term Care Medicaid, or Medicaid’s Buy-In For Working Adults with Disabilities applications if staff have capacity. This position would also help clients with Medicaid Eligibility Appeals if needed and staff have capacity. Incumbents perform screening interviews, assessments, and intake of clients to determine needs; develop, explain, and provide follow-up on action plans; assist the client in completing and submitting applications; coordinates the exchange of information and provision of client services with other agencies; conduct on-site client visits; document program information and outcomes; and provide education to clients on application processes and how to retain public benefits.
This position is grant funded through 8/31/2025. Possible extension if additional funding is received.
About the Position of Community Resource Navigator (Application Assistance)
This class specification represents only the core areas of responsibilities; specific position assignments will vary depending on the needs of the Division.

Provide benefits navigation and application assistance to people who need to apply for SSI, SSDI, LTC Medicaid, AND, and Medicaid’s Buy-In Program for Working Adults with Disabilities if staff have capacity. Can also assist people with Medicaid Eligibility Appeals if staff have capacity. This position will utilize the SOAR model for applying for benefits. This position with work with eligible individuals, with a target population of under 60 years of age and refugee and immigrant groups, as follows: complete a clear action plan, obtain release of information and necessary forms, complete applications, explain benefits and processes, and follow-up throughout the application process to ensure benefits are received (if within grant service period). Depending on continued funding, for the first year after obtaining benefits, this position will monitor and track redeterminations to coach the individual through the necessary steps to maintain benefits.
Performs client screening interviews, intake and assessments to determine service needs and resource requirements; serves as an agency representative with other service providers to address service requirements, makes resource referrals to applicable agencies; provides client advocacy based on required situations.
Evaluates and documents program eligibility status; determines case safety level, emergency status and/or need for crisis intervention or mandatory reporting; determines, develops, and communicates client service action/case plans; monitors and documents plan status and performs follow up on case services; evaluates plan outcomes.
Monitors client status; provides counseling and intervention support; conducts on-site client visits and perform comprehensive needs assessment; monitors services and makes plan adjustments as client needs change; escalates to supervisor as needed and required.
Prepares, documents and maintains case and/or program information; collects, manages and analyzes data to produce reports documenting program measures and outcomes; provides recommendations; ensures the maintenance of timely program or case activity tracking, reporting and applicable billing.
Maintains relationships with agencies to best collaborate in serving clients.
Performs other duties of a similar nature and level as assigned.


Providing face-to-face or telephonic information to clients in need of navigation, education, and application assistance for SSI, SSDI, AND, LTC Medicaid, and Medicaid’s Buy-In Program for Working Adults with Disabilities, as well as Medical Eligibility Appeals.
Working with clients as needed to help them apply for benefits listed above.
Providing consultation and case coordination to clients. This includes the following:

conducting individual needs assessments.
working with clients and their families to identify options to address problems,
completing care plan detailing service needs.
contacting selected agencies to help set up service.
following up with clients to ensure application processes are completed and benefits received.
If service period allows, for the first year after the client obtains benefits, monitor and track specific benefits redeterminations to coach the client, as needed, to maintain their benefits.
Providing interpretation/translation services.




Providing on-site client consultations to refugee and immigrant populations at the Aurora Center for Active Adults and Thronton’s Center for Active Adults.
Age and disability related issues and concerns of adults, their families and caregivers.
Aging and disability area networks and resources.
Knowledge of public benefit eligibility, including for refugee and immigrant populations.
Principles and applications of critical thinking and analysis.
Best practices, trends and emerging technologies.
Principles and practices of program administration and management.
Principles and methods of qualitative and quantitative research.
Modern budget practices.
Record management principles.
Applicable federal, state and local laws, codes, regulations (based on assignment).
Customer service principles.
Specialized equipment relevant to area of assignment.
Modern office technology.
Issues and concerns facing older adults and people who have a disability.
In-depth knowledge of public benefits applicable to this position and how to navigate benefit systems.
Application requirements specifically for SSI, SSDI, AND, LTC Medicaid, and Medicaid’s Buy-In Program for Working Adults with Disabilities, as well as how to appeal Medicaid eligibility.
Advocacy and effective communication techniques in working with Social Security Administration and applicable Colorado governing entities.
Aging network and community resources available in the region.
Familiarity with basic concepts of digital accessibility, including techniques for accessible writing, presenting, remote meetings, and document creation.
Researching industry trends, solutions and best practices.
Performing assessment of client situations to determine needs.
Conducting interviews to determine client or program service needs and/or intervention.
Gathering data, analyzing findings and applying logic and reason.
Interpreting, monitoring and reporting information and statistics.
Authoring original reports, documents and presentations.
Compiling and sorting data and articulating issues and recommendations.

Education and experience requirements:

Bachelor’s degree in gerontology, social work or related human services field and one to three years of related human services experience. OR An equivalent combination of education and experience sufficient to successfully perform the essential duties of the job such as those listed above, unless otherwise subject to any other requirements set forth in law or regulation.
Must have at least one year of experience assisting clients with applying for benefits listed in this job description.
Certification in SSI/SSDI Outreach, Access, and Recovery (SOAR) preferred.
