What you will do: Assess the needs, plan, and deliver care from initial assessment to discharge from services, ensuring the best outcomes for service users. Collaboratively implement and evaluate personalised care plans, working alongside service users, carers and other professionals. Empower and support service users on their recovery journey, helping them regain independence. Work within community settings, advocating for individuals and providing therapeutic interventions as required. Supervise nonqualified staff and ensure high standards of care are maintained. Engage closely with external partners including community services, social services, and other healthcare agencies to provide comprehensive support. Provide essential signposting to additional services to enhance holistic care. Utilise a basic understanding of the Mental Health Act to inform practice.
