The Community & Patient Health Liaison will provide education, health promotion, outreach, and access to resources for patients and non-patients to increase healthcare access. This role will inform the public on services offered at Neighborhood Healthcare (Neighborhood) to reduce healthcare gaps in North County (Escondido, Poway, San Marcos, Valley Parkway). Additionally, this role will work in health centers and in the community to help build trust and awareness for Neighborhood. This role will be the point of contact between various community stakeholders, community-based agencies, and Neighborhood to increase patient growth and make critical linkages to offered programs and services. Must be open to work evenings and weekends to cover community events/meetings. This position requires availability to work evenings and weekends as needed to support community events/meetings. While the standard schedule for this role is 40 hours per week, flexibility is provided and weekday hours can be adjusted when weekend/evening work is required. Responsibilities Leads community promotions and awareness for new sites & services campaigns Presents Neighborhood programs and services presentations to individuals and small groups Serves as the point of contact and advocate to patients and the community regarding information related to Neighborhood programs, services, and active grants Provides Cal-Fresh and health insurance referrals to our Certified Enrollment Counselors for the following programs Covered CA, Medi-Cal, and Children’s Health Insurance Program Provides Neighborhood program/services resources and health education to patients, families, and community members, including planning and leading group sessions and discussions Works with manager to plan and organize health fairs and other public events for local communities Travels to community partner and outreach locations to present site enrollment presentations Maintains, tracks, and reports on outreach program effectiveness after each outreach activity in Asana project management software Makes referrals for patients to health and social service agencies; follows up with referred individuals to ensure services were received Creates interdepartmental referrals for patients in need of medical, dental, behavioral health, and/or other available services Provides excellent customer service to internal and external entities Manage and identifies solutions for difficult patient situations Develops relationships with local healthcare and social service providers to facilitate information sharing and service provisions Maintains excellent relationships with various stakeholders including patients/non- patients, hard to reach vulnerable populations, as community partners Represents Neighborhood in meetings with community partners Meets weekly goals for outreach/engagement targets and prepares monthly quantitative and qualitative reports on outreach/in-reach efforts Assists in completing other program goals and administrative duties assigned, such as answering phone calls and responding to emails Compiles feedback from patient experiences to improve access to healthcare Qualifications Education/Experience High school diploma/GED required One year of outreach, education, and/or customer service experience within a healthcare setting or related field required Experience working with diverse populations required Bilingual (English/Spanish) required Valid CA Driver’s License and proof of auto insurance required Additional Qualifications (Knowledge, Skills and Abilities) Excellent verbal and written communication skills, including superior composition, typing, and proofreading skills Ability to interpret a variety of instructions in written, oral, diagram, or schedule form Ability to assess situations and independently develop a plan of action Knowledgeable about and experience with local community-based agencies Ability to successfully manage multiple tasks simultaneously Excellent planning and organizational ability Ability to work as part of a team as well as independently Ability to work flexible shifts Ability to work with highly confidential information in a professional and ethical manner Physical Requirements Ability to lift/carry 50 lbs/weight Ability to stand for long periods of time Neighborhood Healthcare offers a generous benefit plan that includes: Partially company paid Medical, Dental, and Vision Plans. Compensation Disclosure: The posted salary range reflects the designated pay grade for this position. While this range represents the broader classification of the role, actual compensation will be based on several factors, including but not limited to: the candidate’s overall knowledge, skills, and experience, market data and industry benchmarks, internal equity within the organization, Budgetary considerations and organizational needs. As a result, placement within the range is not guaranteed, and the full pay grade range may not be utilized.
